WHAT DOES IT COVER?

Accidental Damage, Mechanical Failure, and Theft


  1. Applies to all mobile phones owned by the primary account holder

  2. No pre-registration required

  3. Includes cracked screens, liquid spills, burglary and robbery

  4. Pays up to $500 minus a $50 deductible

  5. 2 claims per consecutive 12-month period

  6. Waiting period of 30 days after coverage start date for damage or mechanical failure, no waiting period for theft

  7. Damages that occur during the waiting period are not covered


HOW DO I FILE A CLAIM?*

  • Call or visit your Program Sponsor and let them know you would like to file a claim prior to getting your phone repaired or replaced. Your Program Sponsor will make available the online claims portal.

  • Go to the online claims portal within 90 days of the loss or damage of your covered device.

  • Enter all the required information on the electronic claim form.

  • A claims adjuster will review your submission. If no additional information is needed, you will receive an email with instructions and suggestions on approved repair/replacement facilities where you can take your device locally.

  • If there are questions about your submission, a claims adjuster will call you.

If your claim form is received prior to 3 PM Central Time, you will typically receive a response within 1 business day. If after 3 PM Central Time, you will typically receive a response within 2 business days.
